Definition of malignant residency: Malignant programs make residency miserable compared to non-malignant programs. It may be harsh work environment, dangerous neighborhood, nepotism, financial trouble for the hospital, etc. Many IMG friendly programs fall in to this category so it is not a such a big deal. All AMGs and IMGs (including those with excellent creds) are better off applying and ranking them lower.
Definition of extremely malignant residency: Extremely malignant programs not only make residency extremely miserable but MAY also end medical career of residents by kicking them out. Only 1 or 2 as a pattern get kicked out from these programs every year or so. Only AMGs can afford to NOT rank / apply to such programs. IMGs (including those with excellent creds) are better off ranking them lower after the malignant programs. Be wary when prematch is offered by such programs because that means you may most likely get matched in a better program.
Disclaimer:
Something is better than nothing so it is better to rank these programs including the ones that kick residents atleast towards the end of your rank order. Importantly, trust your own research and your gut feeling first.

1. University of Chicago - Anesthesiology - I received message that the program has extended training of several residents in the last few years and fired one resident who ended up going to the Cleveland Clinic program where they did well.

2. Beth Israel in Boston - OB/GYN - I received message that a resident was terminated due to issues with an attending about 5 years ago and that the resident went to another program and did well.

3. Ochsner General Surgery - post above reports 2 residents were terminated in their 3rd yr of residency d/t failing Step 3 > 2x each and not being able to pass it by the required PGY-3 year.

4. University of Mississippi General Surgery - message received that PGY4 resident dismissed last year, reportedly due to anger management issues.

5. Johns Hopkins General Surgery - PGY3 resident terminated allegedly due to mental health issues. (see link: http://forums.studentdoctor.net/showthread.php?t=660565 )

6. VCU-Shenandoah Valley Family Practice Residency - reportedly PGY1 resident notified of contract non-renewal, resident appealed and was placed on probation, subequently resigned - (see link: http://forums.studentdoctor.net/showthread.php?t=658953 )

7. Cleveland Clinic Neurosurgery - reportedly terminate a resident about every year (see above post)

8. Southern Illinois University Internal Medicine - received message that they terminated a resident.

9. Rapid City Family Medicine - received message that they have terminated a handful of residents over the past 3 or 4 years.

10. Providence Family Medicine in Olympia, WA - received message that they fired a resident last year.

11. Spokane Internal Medicine - received message that they reportedly terminated one last year as well.

12. Univ of Texas San Antonio Internal Medicine - received message that they seems to fire one out of each class or so.

13. Franklin Sq Hospital in Baltimore Internal Medicine - received message that program seems to fire one resident every year.

14. Forbes Family Practice in Monroeville PA - reportedly fired 4 residents in past 3 years - see above post.

15. Indiana University Internal Medicine - received message that the program terminated a resident last year who is now doing well at another program.

16. University of Tennessee Chattanooga - received message that an OB/GYN resident was terminated and at least two other terminations/nonrenewals are pending in other programs.

Extremely malignant programs not only make residency extremely miserable but MAY also end medical career of residents by kicking them out. As everyone knows residency is very, very demanding. Doing one in malignant program can be shattering. Doing one in extremely malignant program (defined as those that kick residents) MAY be career suicide. The unlucky ones in extremely malignant programs are harassed (some times even sexually) and tortured beyond imagination and are in most cases are also forced to see psychiatrist. Worst form of slavery! 

Malignant programs are img friendly and img non-friendly. There are only SOME malignant programs. There are only a FEW extremely malignant programs. Many are lucky and finish even in an extremely malignant program. The sad thing is because they are dependent for a year or so for getting recommendations from the program for job or for fellowship, they most times DONT give true feedback to everyone.

As some one mentioned www.malignantresidency.com's main purpose is to caution others about malignant programs, particularly extremely malignant programs. The submissions we get are long and sad. We remove identifiable information and anything in the lines of slandering and post them in www.maligantresidency.com. Hospitals are invited to counter explain and set things right. Good hospital management understands that residency is not about breeding ego and vanity of unfit faculty but to humanely treat future doctors who are already worked up like slaves. Choose to be NAIVELY OPTIMISTIC and generalize that all residency programs as non-malignant and take the full risk.
Or choose to be some what cautious and rank malignant programs including those that kick residents in the end and reduce some risk.
Or choose to be cautious and skip extremely malignant programs that kick residents but rank other malignant programs and reduce risk.
Or choose to avoid malignant programs if peace of mind is important and your scores / credentials are excellent.
Or choose whatever that fits you in this free world.
